ABSTRACT

Objective:To investigate the risk factors associated with mortality in patients with severe traumatic liver injury (TLI) and to establish and validate an early prediction model for mortality.Methods:A retrospective cohort study was conducted to analyze the clinical data of 273 patients with severe TLI admitted to the ICU from the medical information mart for the intensive care-IV (MIMIC-IV) database. The cohort consisted of 176 males and 97 females, with age ranging from 18 to 83 years [35.6 years(25.7,57.5)years]. The patients were divided into two groups based on in-hospital mortality: the survival group (253 patients, 92.7%) and the death group (20 patients, 7.3%). The two groups were compared with regards to gender, age, cause and type of injury, treatment method, massive blood transfusion, comorbidities as well as vital signs and laboratory tests measured within 24 hours of ICU admission. Univariate analysis was used to screen for risk factors associated with mortality in severe TLI patients. Independent risk factors for mortality were determined using multivariate Logistic regression analysis. Lasso regression was used to screen for predictors of mortality, and a nomogram prognostic model was then established through a multivariate Logistic regression analysis. The area under the receiver operating characteristic (ROC) curve (AUC) was used to evaluate the discrimination of the model, while the Hosmer-Lemeshow goodness-of-fit test and calibration curve were used to evaluate the calibration of the model. The model′s clinical applicability was evaluated through decision curve analysis (DCA). Internal validation was performed by the 200 Bootstrap samples, and external validation was performed by using 163 patients with severe TLI from the emergency ICU collaborative research database (eICU-CRD). Finally, the predictive efficacy of the nomogram model was compared to other trauma or severity scores.Results:Univariate analysis showed that the age, cause of injury, massive blood transfusion, chronic liver disease and laboratory tests measured within 24 hours of ICU admission, including temperature, systolic blood pressure, diastolic blood pressure, mean arterial pressure, shock index, platelets, red blood cell distribution width (RDW), mean red blood cell hemoglobin concentration (MCHC), blood glucose, blood urea nitrogen, creatinine, anion gap, bicarbonate, prothrombin time (PT), activated partial thromboplastin time (APTT) and international normalized ratio (INR) were associated with the mortality of severe TLI patients ( P<0.05 or 0.01). Multivariate Logistic regression analysis revealed that age ( OR=1.08, 95% CI 1.03, 1.12, P<0.01), body temperature <36 ℃ ( OR=8.00, 95% CI 2.17, 29.53, P<0.01), shock index ( OR=9.59, 95% CI 1.76, 52.18, P<0.01) and anion gap ( OR=1.32, 95% CI 1.15, 1.53, P<0.01) were significantly associated with mortality in severe TLI patients. Lasso regression analysis selected 7 predictors, including age, body temperature<36 ℃, shock index, anion gap, chronic liver disease, creatinine and APTT. Based on these 7 predictors, a nomogram prediction model was developed. The AUC of the nomogram for predicting mortality was 0.96 (95% CI 0.94, 0.99), and the Hosmer-Lemeshow goodness-of-fit test indicated a good fit ( P>0.05). The calibration curve demonstrated excellent consistency between the predicted and actual probabilities, and DCA demonstrated that the model had good clinical net benefit at all risk threshold probability ranges. Internal validation confirmed the stability of the model ( AUC=0.96, 95% CI 0.92, 0.98), and external validation demonstrated good generalization ability ( AUC=0.95, 95% CI 0.91, 0.98). Moreover, the nomogram exhibited superior predictive efficacy compared with injury severity score (ISS), revised trauma score (RTS), trauma injury severity score (TRISS), sequential organ failure score (SOFA), acute physiological score III (APS III), Logistic organ dysfunction score (LODS), Oxford acute severity of illness score (OASIS) and simplified acute physiological score II (SAPS II). Conclusions:Age, body temperature <36 ℃, shock index and anion gap are independent risk factors for mortality in severe TLI patients. A nomogram prognosis model based on 7 predictors, namely age, body temperature <36 ℃, shock index, anion gap, chronic liver disease, creatinine and APTT exhibits good predictive efficacy and robustness, and is contributive to accurately assess the risk of mortality in severe TLI patients at an early stage.

ABSTRACT

OBJECTIVE To study the effects of Hugan buzure formula (HBF) on intrahepatic cholestatic liver injury in rats and its potential mechanism. METHODS Rats were randomly divided into control group, model group, ursodeoxycholic acid (UDCA) group (positive control, 60 mg/kg ) and HBF low-dose, middle-dose and high-dose groups (HBF-L, HBF-M, HBF-H groups, 0.4, 0.8, 1.6 g/kg ), with 6 rats in each group. The rats in each drug group were given the corresponding drug solution intragastrically, once a day, for 7 consecutive days. The rats in the control group and the model group were given equal volumes of water intragastrically. On the 5th day, except for the control group, the rats in other groups were single intragastrically administered with alpha-naphthyl isothiocyanate olive oil solution (100 mg/kg) to establish the model. After 48 h of modeling, the contents of liver function indexes (aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, total bile acid, total bilirubin, direct bilirubin) and oxidative stress indexes [malondialdehyde (MDA), glutathione (GSH), superoxide dismutase] in serum of rats were detected; the pathological changes of liver tissue were observed. The mRNA expressions of inflammation-related factors [tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β)] and farnesoid X receptor (FXR) signaling pathway-related factors [FXR, small heterodimer partner (SHP), multidrug resistance protein 2 (MRP2), bile salt export pump (BSEP), Na+-taurocholate cotransporting polypeptide (NTCP), organic anion-transporting polypeptide 2 (OATP2) and cholesterol 7α-hydroxylase (CYP7A1)], the expressions of FXR signaling pathway-related proteins (FXR, MRP2, BSEP, NTCP) and nuclear factor- κB p65 (NF- κB p65) in liver tissue were detected.RESULTS Compared with the model group, the contents of liver function indexes and the level of MDA in serum, the mRNA expressions of the above inflammation-related factors and CYP7A1, and the relative expression of NF-κB p65 in liver tissue were significantly decreased; the levels of GSH in serum, the mRNA expressions of FXR, SHP, MRP2, BSEP, NTCP and OATP2, and the relative expressions of FXR, MRP2, BSEP and NTCP in liver tissue were significantly increased (P<0.05 or P<0.01); the pathological changes of liver tissue were significantly improved. Only some indexes in HBF-L group, HBF-M group and UDCA group were significantly reversed (P<0.05 or P<0.01). CONCLUSIONS HBF can prevent intrahepatic cholestatic liver injury in rats, and the effects may be related to the activation of FXR signaling pathway and the reduction of inflammation and oxidative stress.

ABSTRACT

Hepatocellular carcinoma (HCC) often has an insidious onset, and most patients are in the advanced stage and have lost the best opportunity for treatment at the time of diagnosis, resulting in a poor prognosis. As a multifunctional transcription factor, Yin Yang 1 (YY1) regulates the transcription of a variety of important genes and plays a key role in various tumors. Previous studies have shown that YY1 affects many biological behaviors such as proliferation, apoptosis, migration, and angiogenesis and is closely associated with drug resistance and poor prognosis of HCC. This article reviews the research advances in the role of YY1 in the development and progression of HCC, so as to provide a theoretical basis for the treatment of HCC.

ABSTRACT

Objective To investigate the risk factors for covert hepatic encephalopathy (CHE) in patients with liver cirrhosis and their influence on prognosis. Methods A total of 416 patients with liver cirrhosis who were hospitalized in a grade A tertiary hospital in Chongqing from September 2019 to June 2020 were enrolled in the study, and according to the presence or absence of CHE, they were divided into CHE group with 212 patients and non-CHE group with 204 patients. Clinical data and laboratory examination results were collected, and follow-up was performed for 6 months. The t -test was used for comparison of normally distributed continuous data between groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between groups; the chi-square test, the continuous correction chi-square test, and the Mann-Whitney U test were used for comparison of categorical data between groups. Univariate and multivariate logistic regression analyses were used to analyze the risk factors for CHE. Results The incidence rate of CHE was 51%. The univariate analysis showed that age, course of disease, the medical history of hepatic encephalopathy (HE), infection, ascites, electrolyte disturbance, hepatorenal syndrome, Child-Pugh class, prothrombin time, total bilirubin, creatinine, platelet, prothrombin activity, albumin, and Model for End-stage Liver Disease (MELD) score were the influencing factors for CHE (all P < 0.05). The multivariate logistic regression analysis showed that the medical history of HE ( OR =10.848, 95% CI : 4.971-23.674, P < 0.05), transjugular intrahepatic portosystemic shunt (TIPS) ( OR =4.334, 95% CI : 1.203-15.621, P < 0.05), Child-Pugh class ( OR =4.968, 95% CI : 1.299-18.992, P < 0.05), and MELD score ( OR =1.253, 95% CI : 1.161-1.352, P < 0.05) were independent predictive factors for CHE ( P < 0.05). The follow-up study showed that CHE had an effect on the short-or medium-term readmission, HE, and death of patients (all P < 0.05). Conclusion CHE has a relatively high incidence rate and greatly affects the prognosis of patients with liver cirrhosis. The development of CHE should be taken seriously in patients with a past history of HE, a history of TIPS, Child-Pugh class C liver function, and a high MELD score, and identification, screening, and intervention should be performed as early as possible to improve the prognosis of patients with liver cirrhosis.

ABSTRACT

Objective:To explore the application value of heuristic teaching combined with anchored instruction in nursing teaching in emergency department.Methods:A total of 80 nursing students assigned to the Emergency Department of The First Affiliated Hospital of Air Force Medical University from July 2019 to October 2020 were divided into control group and research group, with 40 cases in each group. The control group used traditional teaching, and the research group used heuristic teaching combined with anchored instruction. At the end of the practice, the teaching effect was evaluated through the examination results (theoretical examination and scenario simulation practice skills examination) and questionnaire survey of teaching quality and teaching satisfaction. SPSS 22.0 was used for t test and chi-square test. Results:The theoretical examination scores [(93.20±2.88) vs. (90.53±3.06)] and the practical skill examination scores (basic operation examination, specialized operation examination and comprehensive quality) of the study group were higher than those of the control group, and the difference was statistically significant ( P<0.05). The overall situation of teaching quality and teaching satisfaction of nursing students in the study group was significantly higher than that in the control group, and the difference was statistically significant ( P<0.05). Conclusion:Heuristic teaching combined with anchored instruction is helpful to improve the theoretical knowledge, practical skills, operation level and comprehensive quality of nursing students, and has good popularization value in clinical nursing teaching.

In recent years, driven by the support of national policies and societal needs for employments, talents in biology majors have been growing rapidly. To foster high-calibre biology talents for the society in the context of the "double world-class initiative" in higher education, this study analyzed the opinion of biology undergraduates in Huzhou University on employment and their professional recognition of biology majors. The aim of this study was to propose a high-quality employments-driven talent training mode for undergraduates in biology majors, so as to serve as a reference for the reform in training modes of other relevant majors.

ABSTRACT

Objective:To evaluate the value of systematic education for primiparas undergoing epidural labor analgesia.Methods:A total of 240 primiparas who were suitable and voluntarily requested epidural labor analgesia, aged 20-35 yr, at 37-41 + 6 weeks gestation, with a singleton fetus, in vertex presentation without fetal abnormality, were selected.According to whether the primiparas and their accompanying family members had received systematic education on epidural labor analgesia during pregnancy, the primiparas were divided into 2 groups ( n=120 each): education group and control group.When the uterine contraction was regular and the diameter of the uterine orifice reached 2 cm or more, epidural labor analgesia was carried out.Before the begining of epidural puncture, the degree of anxiety of pregnant women was evaluated using the self-rating anxiety scale.The time of communication before analgesia, time of placing body position, and satisfaction of puerperae and their family members with labor analgesia were observed and recorded. Results:Compared with control group, the self-rating anxiety scale score was significantly decreased, the time of communication before analgesia and time of placing body position were shortened, and the rate of satisfaction of puerperae and their family members with labor analgesia was increased in education group ( P<0.05). Conclusions:Systematic education is helpful in relieving the anxiety of primiparas during epidural labor analgesia, increasing the efficiency of analgesia implementation and improving the delivery experience of puerperae.

ABSTRACT

Objective:To investigate the effects of forced-air warming blankets combined with conventional warming strategies on the quality of postoperative recovery and cognitive recovery in elderly patients undergoing laparoscopic radical resection of colorectal cancer.Methods:This was a prospective, randomized, controlled trial.A total of 70 patients aged 65-75 years, who were classified as American Society of Anesthesiologists(ASA)grade-Ⅱ or Ⅲ, undergoing the laparoscopic radical resection of colorectal cancer under general anesthesia, were enrolled.Patients were randomly divided into two groups: the forced-air warming group(group FAW, n=35)and the conventional warming group(group CW, n=35). The inadvertent perioperative hypothermia(IPH), postoperative shivering, postoperative agitation, anesthesia recovery time and other postoperative complications were compared between the two groups.The post-operative quality of recovery scale(PQRS)was used to evaluate the quality of postoperative recovery and the recovery of cognitive function before and 1, 3, and 7 days after operation.Results:As compared with the group CW, the group FAW showed that the incidences of IPH, postoperative shivering and agitation were decreased(5.7% vs.22.8%, 2.8% vs.28.6%, 5.7% vs.31.4%, χ2=4.200, 10.057 and 7.652, P=0.042, 0.003 and 0.006), and the satisfaction degree of patients was increased at 48 hours after operation( P<0.01). The postoperative wake-up time was prolonged in the group CW as compared with the group FAW( P<0.01). Compared with the group CW, the proportion of patients with excellent overall recovery quality was increased in the group FAW 1 day after surgery( P<0.05). There was no significant difference in general recovery quality and cognitive recovery between the two groups at 3 and 7 days after operation. Conclusions:For elderly patients undergoing laparoscopic radical resection of colorectal cancer, the forced-air warming blanket combined with conventional warming strategies is more helpful to decrease the incidence of IPH, reduce postoperative shivering and improve the overall recovery quality at 1 day after operation, but no significant effect on postoperative recovery of cognitive function is found.

ABSTRACT

Objective:To investigate the correlation between inositol level and glycolipid metabolism in gravidas with gestational diabetes mellitus (GDM).Methods:A cross-sectional study was conducted on 80 GDM cases undergoing routine examination at Fujian Provincial Maternity and Children's Hospital from November 2018 to August 2019, who were selected as GDM groups. Another 50 women with uncomplicated pregnancies during the same period were selected as the control group. Blood and urine inositol level and serum glycolipid profiles were compared between the two groups, and their association was analyzed. Independent or paired-sample t test, Mann-Whitney test, Chi-square (or Fisher's exact) test, and Pearson correlation test were performed for statistical analysis. Results:The serum inositol concentration and high-density lipoprotein (HDL) cholesterol in the GDM group were significantly lower [322.1 ng/ml (279.1-364.1 ng/ml) vs 403.8 ng/ml (391.8-425.3 ng/ml), Z=-7.879; 1.8 mmol/L (1.5-2.0 mmol/L) vs 2.0 mmol/L (1.7-2.2 mmol/L), Z=-2.419; both P<0.05], while the concentration of urine inositol, lipoprotein-a (lipo-a), 0 h-, 1 h-, 2 h-oral glucose tolerance test (OGTT) glucose, fasting insulin, and glycosylated hemoglobin (HbA1c) were significantly higher when comparing to the control group [192.2 ng/ml (171.0-219.9 ng/ml) vs 143.8 ng/ml (121.1-158.6 ng/ml), Z=-6.834; 253.2 mg/L (65.0-349.0 mg/L) vs 148.5 mg/L(46.5-159.3 mg/L), Z=-0.187; 5.0 mmol/L (5.1-5.6 mmol/L) vs 4.4 mmol/L (4.2-4.6 mmol/L), Z=-5.547; 10.0 mmol/L (9.1-11.3 mmol/L) vs 7.8 mmol/L (7.0-8.4 mmol/L), Z=-6.987; 8.6 mmol/L(7.6-9.4 mmol/L) and 6.6 mmol/L (5.7-7.1 mmol/L), Z=-7.100; 18.2 mU/L(10.6-25.9 mU/L) vs 11.0 mU/L (6.3-12.7 mU/L), Z=-4.537; 5.4%(4.5%-5.5%) vs 5.1%(4.9%-5.4%), Z=-3.468; all P<0.05]. (2) Serum inositol concentration was negatively correlated with fasting insulin and 0 h-, 1 h-, 2 h- OGTT glucose level ( r=-0.386, -0.416, -0.350 and -0.407, respectively); urinary inositol concentration was positively correlated with 0 h-, 1 h-, 2 h-OGTT glucose levels ( r=0.402, 0.389 and 0.429, respectively) (all P<0.05). Conclusions:Serum inositol concentration was decreased, and urinary inositol excretion was increased in women with GDM. Measurement of changes in inositol levels during the second trimester may be helpful to assess the metabolic status of pregnant women.

ABSTRACT

Objective:To investigate the correlation between concomitant diabetes mellitus and clinical features in patients with COVID-19.Methods:Fifty patients with COVID-19 admitted to the Sixth People′s Hospital of Shenyang were divided into 2 groups with and without diabetes mellitus. Indexes such as hospitalization days, duration of positive virus-PCR result, secondary infection, duration of fever, rate of liver dysfunction, and rate of cardiac injury were compared between 2 groups.Results:The average hospitalization durations of diabetes group and control group were(24.0±6.8) and(18.4±5.9) days, respectively, and the duration of positive virus-PCR testing results were(16.0±4.5) and(12.3±4.4) days respectively, suggesting longer hospital stay and longer time for virus clearance in the diabetes group than those in the control group(both P<0.05). The rates of secondary infection, severe subtype, liver dysfunction and cardiac injury in the diabetes group increased, though without significant difference( P>0.05). Conclusion:Prolonged duration of positive virus-PCR result and average days of hospitalization in patients with COVID-19 may be associated with diabetes mellitus.

Objective:To investigate differentially expressed genes in muscular tissue in Alzheimer's disease mice and normal mice, and to elucidate the role of differentially expressed genes in the pathogenesis of Alzheimer's disease.Methods:Muscle tissues of Alzheimer’s disease mice and normal mice were collected and the second-generation high-throughput RNA transcription sequencing(RNA-seq)technique was used to screen differentially expressed genes.Additionally, Gene Ontology(GO)functional annotation and Kyoto encyclopedia of genes and genomes(KEGG)pathway analysis were conducted.Differences in the expression of candidate genes in muscle tissues between Alzheimer’s mice and normal mice were verified by real-time fluorescence quantitative polymerase chain reaction(qRT-PCR).Results:A total of 4 222 genes in muscles had different expression between Alzheimer's disease mice and normal mice, with 2087 genes highly expressed and 2 135 lowly expressed in muscles of Alzheimer's disease mice.GO analysis showed that significantly up-regulated functional genes were mainly enriched in cellular, single biological, metabolic and catalytic processes and activities.KEGG pathway analysis showed that differentially expressed genes were significantly enriched in signal pathways related to metabolism and inflammation.Results from qRT-PCR showed that the mRNA expression of candidate genes was consistent with the trend of transcriptome sequencing results.Conclusions:Differentially expressed genes and related important regulatory signal pathways were screened at the transcriptome level in muscle tissues of Alzheimer's disease mice and normal mice.The differentially expressed genes are enriched in signal pathways related to metabolism and inflammation in muscle tissues of Alzheimer's disease mice and the findings provide an important basis for further exploration of the pathogenesis of Alzheimer's disease.

ABSTRACT

Extracellular vesicles (EVs) refer to bilayer membrane transport vesicles secreted by cells. EVs can take macromolecules from cells and transfer them to receptor cells. Among these macromolecular substances, the most studied are microRNAs (miRNAs). miRNA is non-coding RNA involved in the regulation of gene expression. It has been confirmed that there are different non-coding RNAs in mammalian follicular fluid EVs. EVs carrying miRNA can act as an alternative mechanism for autocrine and paracrine, affecting follicular development. This paper systematically introduced the kinds, characteristics and methods of isolation and identification of EVs, focusing on the effects of EVs and miRNAs on follicular development, including early follicular development, oocyte maturation, follicular dominance and effects on granulosa cell function. At the same time, the authors prospected the future research of EVs and microRNAs in follicular fluid, and provided ideas and directions for the research and application of EVs and miRNA functions in follicular fluid.

ABSTRACT

Background: Irritable bowel syndrome (IBS) is a commonly seen functional bowel disorder in clinical practice. Previous researches demonstrated that early adverse life events (EALs) were associated with the development of IBS. However, there are few researches focused on this field in China. Aims: To explore the effect of childhood abuse, a type of EALs, on the development of IBS in adults. Methods: Fifty-six IBS patients and 50 healthy subjects in Tongji Hospital of Tongji University from Oct. 2017 to Apr. 2018 were enrolled. The EALs and quality of life of these two groups were evaluated using Childhood Trauma Questionnaire (CTQ) and the MOS 36-Item Short-Form Health Survey (SF-36) scale. Furthermore, Irritable Bowel Syndrome Severity Scoring System (IBS-SSS), SF-36 and Irritable Bowel Syndrome Quality Of Life (IBS-QOL) scales were compared between IBS patients with and without childhood abuse. Results: The CTQ score and incidence of childhood abuse were significantly higher in IBS group than in control group (P<0.05), while the scores of bodily pain, general health, vitality, social functioning and mental health in SF-36 were significantly lower in IBS group than in control group (P<0.05). Compared with patients without childhood abuse, IBS patients with childhood abuse had a higher IBS-SSS score (P<0.05); the physical functioning, general health, vitality, social functioning and mental health in SF-36 and the dysphoria, interference with activity, body image, health worry, social reaction, sexual intercourse and relationship in IBS-QOL were poorer in patients with childhood abuse than those without (P<0.05). Conclusions: Experience of EALs plays an important role in the development of IBS and might affect the symptom severity, quality of life and social function in adult IBS.

Objective To study the prevalence of ischemic cardiocerebrovascular disease (ICVD) and its risk factors in Chengdu pediatric healthcare people.Methods A total of 822 pediatric healthcare people in our hospital were divided into coronary heart disease (CHD) group (n=43) and CHD-free group (n=779).Their consecutive data from 2011 to 2016 were retrospectively analyzed.The risk factors for ICVD were analyzed by multivariate regression analysis.Results CHD was detected in 523 out of 100 000 cases in 5 years and in 104 out of 100 000 cases in each year.The age was significantly older,the incidence of DM and the serum FBG and Cr levels were significantly higher in CHD group than in CHD-free group (71.26±9.61 vs 63.81±9.79,P=0.000;20.5% vs 8.2%,P=0.045;6.38±2.48 mmol/L vs 5.09±1.10 mmol/L,P=0.000;80.47± 20.87 μmol/L vs 74.81 ± 15.10 μmol/L,P =0.020).Logistic regression analysis showed that age,smoking,and serum FBG level were closely associated with ICVD (OR=1.104,95%CI:1.057-1.154,P=0.000;OR=2.219,95%CI:1.018-4.835,P=0.045;OR=1.564,95% CI:1.320-1.852,P =0.000).Conclusion The prevalence of ICVD is relatively low in pediatric healthcare people,which is closely associated with age,smoking and hyperglycemia.

ABSTRACT

Objective This study investigated the effect of tongxinluo (TXL) on high glucose-induced proliferation and apoptosis of cardiac fibroblasts (CFs),to explore the possible mechanism by which TXL inhibits myocardial fibrosis. Methods Primary culture and subculture of neonatal SD rat CFs was carried out as follows. Immunofluorescence staining was performed to identify CFs. The CFs were divided into control group (cultured with low-glucose DMEM),model group (cultured with high-glucose DMEM),and TXL treatment groups (cultured with high-glucose DMEM+TXL at 20,80,and 320 μg/mL). The proliferation of CFs in each group was detected by MTT assay. The expression of collagen typesⅠand Ⅲ in each group was detected by ELISA. Western blotting was used to detect the expression of bax and bcl-2 in each group. Results CFs Proliferation and collagen typesⅠandⅢsecretion were higher in the model group than in the control group. The CFs proliferation and collagen content in the TXL treatment groups were significantly lower than those in the model group. bcl-2 expression was significantly increased and bax expression was decreased in the model group,compared with the corresponding values in the control group. In comparison with the model group,bcl-2 expression was downregulated and bax expression was upregulated in the TXL treatment groups. Conclusion TXL can reduce the CFs proliferation and inhibit their collagen secretion under high glucose conditions. TXL also induces the CFs apoptosis by reducing bcl-2 expression and increasing bax expression. Thus,TXL can play a role in the treatment of myocardial fibrosis.

ABSTRACT

Objective To explore the risk factors for gestational diabetes mellitus (GDM ) in advanced maternal age (AMA) at the early stage of pregnancy .Methods A total of 1200 subjects with AMA were enrolled in this study as AMA group from Jun 2016 to Jul 2017 in Beijing Obstetrics and Gynecology Hospital of Capital Medical University .Each group was further divided according to OGTT results :GDM group and NGDM group .The risk factors of GDM were analyzed .Results The prevalence of GDM was 24.17%(290/1200) .The history of type 2 diabetes in first-degree relatives,pre-pregnancy BMI,TG,FPG,FIns,and HOMA-IR were significantly different in the two groups (P<0.05) .Pearson correlation analysis showed that GDM was positively correlated with the history of type 2 diabetes in first-degree relatives,pre-pregnancy BMI,TG,FPG,FIns and HOMA-IR (r= 0.519,0.413,0.451, 0.625,0.531,0.214,P< 0.05) .Multivariate logistic regression analysis showed that history of type 2 diabetes in first-degree relatives,pre-pregnancy BMI,TG,FPG,FIns and HOMA-IR were independent risk factors for GDM (OR=1.635,1.133,2.153,2.418,1.390,1.901,P<0.05) .Conclusion During early pregnancy in subjects with AMA,TG,FPG,FIns,history of type 2 diabetes in first-degree relatives,pre-pregnancy BMI were independent risk factors for GDM .

Objective@#To investigate work-related musculoskeletal disorders (WMSDs) of workover workers in an oilfield and explore the influencing factors.@*Methods@#In June 2016, 685 workover workers were selected as working group, and 191 other workers exposed to toxic and harmful operations were selected as control group. Cross sectional investigation was conducted by self-made questionnaire, and the influencing factors of musculoskeletal disorders were analyzed by unconditional multivariate Logistic regression.@*Results@#The total annual prevalence of WMSDs for workover workers was 90.5%, with the highest prevalence of low back pain of 71.5%. The prevalence of WMSDs increased with the increase of service age, and the difference between the drinkers was higher than that of the non drinkers (P<0.05) . Multivariate Logistic regression analysis showed that working age, drinking and the posture of lifting heavy objects were risk factors for WMSDs in workover workers.@*Conclusion@#The WMSDs appears to be a serious problem in oilfield workover workers. Corresponding measures should be taken to prevent the risk factors and control the occurrence of diseases effectively.

ABSTRACT

The consecutive data of 822 senior public officials in Chengdu undergoing health checkup from 2011 to 2016 were retrospectively reviewed.Among them,56 new cases of diabetes was diagnosed with a cumulative incidence of 6.81%.Fifty six age-and sex-matched healthy subjects served as controls,the risk factors of new-onset diabetes were analyzed with multivariate logistic regression.The results showed that BMI (OR =1.82,95% CI:1.27-2.59,P =0.00) and fasting plasma glucose (OR =13.63,95% CI:2.71-68.43,P =0.00) were independent risk factors of new-onset diabetes in senior public officials.

ABSTRACT

Objective To analyze the compatibility of bone marrow mesenchymal stem cells (BMSCs) with amphiphilic peptide three-dimensional gel.Methods Three healthy 3-week old SD rats were taken for separating femur and tibia to obtain BMSCs,the BMSCs surface antigen was detected by flow cytometry;the 10 mg/mL RGD-cyclic amphiphilic peptide solution was added into the same volume of DMEM/F12 culture medium,after a few seconds,which was self assemble into three-dimensional gel.Three dimensional gel structure was observed by transmission electron microscope (TEM).1 × 106 cells/mL BMSCs suspension and RGDcyclic amphiphilic peptide were mixed to form a 3D culture system,1 × 106 cells/mL BMSCs suspension was mixed with polylysine to form a 2D culture system,the serum-free culture was conducted;the CCK-8 method was used to observe the cell growth situation,calcein acetoxy methyl ester/propidium iodide (PI) double standard staining was performed.The effect of RGD-cyclic amphiphilic peptide on the proliferation of BMSCs was observed by fluorescence microscopy.Results The separated and cultured BMSCs highly expressed CD29 and CD90,but lowly expressed or did not express CD34 and CD45;TEM showed that the gel was composed of multiple empty nanofibers with the nanofiber diameter of 2-5 nm and length of 100-1 000 nm;the molecular weight of synthetic peptides detected by mass spectrometry (MS) was 1 256.37,which was consistent with the theoretical value;the HPLC analysis showed that RGD-amphiphilic peptide purity was 95.88 %;the calcein acetoxyl methyl ester/PI double staining showed that in the 3D culture system,a few BMSCs died after 30 min and the cells began to proliferate after 12 h,the proliferation was more active than that of 2D culture,and the difference was statistically significant (P<0.05);CCK-8 cell count showed that the proliferation activity of 3D culture system was higher than that of 2D culture system,and the difference was statistically significant (P<0.05).Conclusion RGD amphiphilic peptide has a good biocompatibility with BMSCs,and may become the tissue engineering scaffold material.
